1. PURPOSE
1.01 The purpose of this Statement of Policy and Procedure is to provide MakeWay Foundation staff with guidance in regard to COVID-19 vaccination requirements.
2. SCOPE
2.01 This policy applies to all MakeWay Foundation staff planning to work from a MakeWay office or attend any MakeWay events.
3. POLICY
3.01 MakeWay Foundation is not currently mandating vaccinations from employees; however, any staff intending to work form a MakeWay office must be double vaccinated. Please note: this is not a mandatory requirement as staff are still able to continue remote work for the foreseeable future.
3.02 MakeWay Foundation Staff must voluntarily disclose their vaccination status –and send proof of status -if they are planning to go into the office or attend any MakeWay events. This will allow MakeWay to continue to ensure the health and safety of team members while allowing more staff to use the MakeWay offices as desired.
3.03 If MakeWay staff go into the office or attend an event and are not able, or are choosing not to, obtain a COVID-19 vaccine they will be required to complete a rapid test and submit a negative test result a minimum of one (1) hour prior to accessing the building.
3.04 MakeWay will maintain vaccination disclosure information, including documentation verifying receipt of a vaccination series approved by Health Canada or the World Health Organization, in accordance with privacy procedures outlined in 3.05of this policy. This information will only be used to the extent necessary for implementation of this guidance, for administering health and safety protocols, and infection and prevention control measures in the workplace.
3.05 Privacy Procedures:
- Staff must email their vaccine receipts to Cristina at [email protected] within one week of commencing employment with MakeWay.
- Proof of vaccination will be stored in a password protected folder, kept on the MakeWay Box server. Access is limited to Cristina and Director, PEL (Erin).
- Each office will have a designated “point person”. Each point person will be notified within one (1) week of any staff in their office/regional area who have not voluntarily disclosed their status simply to ensure these folks are communicating to them and given access to rapid tests if they are planning to be in the office. This list of staff will not be shared outside of our office point people and PEL.
- The point people for each office are:
- Toronto –Erin
- Yellowknife & surrounding –Steve
- Winnipeg –Julie
- Vancouver – Danae
4. RESPONSIBILITY
4.01 It is the responsibility of MakeWay staff to send proof of vaccination to Cristina within one (1) week of commencing employment at MakeWay.
4.02 It is the responsibility of PEL to store proof of vaccination in a password protected folder and communicate to each point person which staff have not voluntarily disclosed their vaccination status.
4.03 It is the responsibility of each point person to ensure staff that have not voluntarily disclosed their vaccination status complete a rapid test prior to entering the office.
4.04 It is the responsibility of all staff who do not wish to disclose their vaccination status to complete a rapid test one (1) hour prior to entering a MakeWay office.
4.05 It is the responsibility of PEL to support offices to obtain additional testing kits on an as needed basis.
4.06 It is the responsibility of the Director, PEL to review this policy at least annually and update it as necessary to reflect Public Health Orders and other regulatory requirements.
5. DEFINITIONS
5.01 “Proof of Vaccination” means documentation verifying receipt of a vaccination series approved by Health Canada or the World Health Organization.
